What this study is about

This trial is testing whether telephone coaching sessions, called Talking About Cancer (TAC), can help people with advanced cancer and their families talk about end-of-life care. The goal is to improve participation in advance care planning, which includes having conversations about EOL care and completing documents like a living will. TAC involves phone calls from a social worker to guide patients and caregivers through these important discussions.
