What this study is about

Researchers are testing if a palliative care intervention called ALIGN can be delivered to older adults with advanced cancer who are discharged from the hospital to a skilled nursing facility. The trial will assess whether this intervention can be successfully provided and how caregivers can best make medical decisions for loved ones when they cannot do so themselves.
